I like the look of my breasts and nipples when my nipples are erect, but when they are not they do not protrude from my areola at all and appear flat. However I've ready that they're not truly "flat" since they protrude when cold or stimulated. Is this normal because I feel like I never see that on other women. I have small breasts anyway so I wonder if this could be helped with an augmentation? Otherwise I'm curious about less invasive options.

Any sudden change in breasts or nipples should be followed up by your private physician or GYN. It is rare for serious issues to arise in both breasts at the same time. Hormonal changes can certainly cause bilateral issues.

The permanent stitch can be released. It is unlikely that at 3 years post op the scar will widen. You can then undergo a repeat donut mastopexy. However, you have asymmetrical nipple-areola complexes. You would benefit from revising both nipple-areola complexes.
