Currently 605cc high profile breast implants - How can I make my breasts look nice and big, but not oversized? (Photos)

Question

Naturally, I was a 34 A cup size. Now I have 605 cc high profile breast implants. I am 44 years old, 5’4” and 120 lbs. I feel I have wider hips, and like to have an hourglass figure however, I feel my current implants are oversized on me and are working against me looking too and will eventually sag more and make me look less attractive. They don’t shake at all definitely don’t look real. I want to have big boobs, maybe size C cup and for them to look more attractive. Can you help me know what size implants I should use and should they be high or medium profile? Any suggestions on the shape, size and style? I would also love for them to be able to shake if possible, mainly I just want to have big boobs that fit me. I am really looking forward to hearing your professional advice. You will get a great result if you make modest changes. One factor I would consider is how long your implants have been in place. To go from an A cup to C cup with 605cc implants is a significant change. If your implants have been in for place for years, then the breast tissue and skin will obviously be somewhat stretched out over that course of time. If you reduce the size of the implants modestly, then your surgeon can tighten the capsule that's around the implant now using a technique like popcorn capsulorrhaphy. If you reduce the size of the implant more significantly, then the surgeon will need to tighten the pocket more aggressively, possibly using by doing a suture capsulorrhaphy.
It also depends on what you consider to be an "attractive" breast. If you change from a high profile to a moderate profile implant, you aren't going to have as much forward projection with the implant and may achieve a little more fullness at the top of the breast (because a moderate profile implant is wider than a high profile implant).
To accomplish the 'shaking' that you desire, it may be worth considering an implant that is less cohesive. A less cohesive implant is softer and more closely mimics natural breast tissue, so you don't get a much upper pole roundness, but the implant assumes more of a natural teardrop shape and moves more similarly to natural breast tissue.

Generally, speaking breast documentation outcomes are based on three variables.
The first is the patient candidacy for the procedure.
The second is implant selection.
The third variable is the surgeon’s ability to put the implants in the anatomically correct position(doing the procedure well). 
In your case, the single variable that is off was implant selection.
From your pictures, it appears you were an excellent candidate for augmentation and the surgical technique was superb and your surgeon should be commended.

Implant selection is a challenging process.
Each plastic surgeon will have their own way of doing it.
Some plastic surgeons have the patient make the final decision for a specific implant other surgeons will make the decision based on input from the patient.
Some surgeons do this better than others.
My personal method has been to work with before and after pictures to get an understanding of what the patient is hoping to achieve.
 I bring those pictures with me to use as a reference during surgery.
During the procedure I use temporary implant sizers to determine what size implant will give the desired outcome.
I typically have a full selection of implants available at my disposal so I can choose the implant that gives the the corresponding outcome.
I don’t think pre-selecting implants is sufficient.
Using this technique has some drawbacks.
When the surgeon makes the final decision, then they are responsible for that decision.
If the patient makes the decision and the patient is on the hook for having made the call.
 Making the decision during surgery also requires maintaining an implant in inventory or sending back implants that weren’t used.
Generally, speaking, using excessively large implants will lead to higher complication rates and higher rates of revision surgery.
We should count your blessings that your surgeon did a good job. 
Downsizing Kan lead to undesirable aesthetic outcomes if the skin and breast tissue has been stretched excessively.
I think in your case, you should be able to downsize and still have a really good outcome.
I would probably lean towards a lower profile implant, but the decision will ultimately be up to you and your surgeon.
 Since your first surgeon did a great job except for the implant size I suggest you go back to him or her.
If they’re not available, then consult with a few other plastic surgeons in your community to see what they have to say.
Swapping implants is technically an easier procedure than a breast augmentation and you may be able to negotiate for a slightly better price, especially if you go back to your original surgeon.
Each provider will have their own view on this.
In a nutshell, I think downsizing will be in your favor and you have the potential for a very good outcome.
