I am planning to upgrade my small (280cc) implants since I am not as skinny now and they look narrow now on my size 8 frame. I also want a sexier look. My doctor recommended 450cc to achieve the look similar to the photos I showed him, however, I am concerned because I love clothes and want to be able to wear things "off the rack" (without tailoring). What is the breast or implant size beyond which it would typically be difficult to buy clothing that fits properly?

This will not get better with time. Although Dr. Baxter dutifully gives the various reasons why it can happen, the overwhelming reason is that the implant chosen was too wide (tall) for the distance between your nipple an original inframammary crease, and not incomplete muscle release.
Your surgeon who has measured you will be the best one to tell you if a higher profile is needed for the larger breast. It is best that the implant sits well in the pocket. Going larger often means using a higher profile implant with a more narrow base to fit the base of your breast.
