Hello and thank you for your question. As long as your cell counts are well controlled and you are able to obtain medical clearance from your primary physician, then you may be a candidate for surgery.  Based on your photograph, you are a
better candidate for waistline contouring liposuction of your abdomen and flanks.  This can improve your overall shape with a
smaller waist.  Make sure you specifically
look at before and after pictures of real patients who have had this
surgery performed by your surgeon and evaluate their results.  The most
important aspect is to find a surgeon you are comfortable with. I recommend
that you seek consultation with a qualified board-certified plastic surgeon who
can evaluate you in person.

Great candidate for liposuction as far as the volume is concerned.  Once you can get medical clearance I think your results will look quite good.  You should make sure that you see your infectious disease doctor and make sure your PS is aware of your HIV status.
