I am getting my implants removed via enbloc and my surgeon offers excess skin excision done by making a football shape cut under nipple to help lift the breast. She also offers areola adjustment but says it is not necessarily needed. My question is would other doctors recommend the areola adjustment? I have 375cc silicon implants being removed and am 5 feet tall 200 pounds so have significant natural breast tissue.

En bloc surgery is for cancer surgery only and does not refer to removing implants or the capsules. It is much more involved and something you probably will not have. Removal with the capsules may be similar to your first procedure in pain and recovery.
Enbloc surgery is for cancer surgery only, and includes removing surrounding tissue to get clear margins away from cancer. A total capsulectomy can be done to remove the capsule and its contents including the implant and any fluid. You would have a total capsulectomy.
En bloc capsulectomy is a significantly riskier operation with a longer recovery, and is rarely necessary. A total capsulectomy can be done without using the en bloc method, and even if a partial capsulectomy is done it is highly unlikely that any parts left behind would cause issues.
