Want round, full, big, perky breasts - what do I need done? (photos)

Hi, thank you for sharing your question. You are a great candidate for breast augmentation. You may have a small amount of glandular ptosis which can likely be correct with a dual plane approach. The proper selection of implants is best done in person after an evaluation by a board certified plastic surgeon because a number of measurements are required. You will be able to achieve your goals. Thank you for your question. Unfortunately I can't see you wish pics on this end but based on the photo there is moderate breast sagging. It is important to look at before/after photos of people with your same appearance to get a better sense if you would be happy with implants alone. Your surgeon can make recommendations about the implant size, profile and type based on your breast dimensions and goals.

I recommend an in person evaluation with a plastic surgeon to be measured and discuss your goals. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. Women tell me that this process is very helpful in determining what size and style breast implant is right for them. Mildly Sagging Breasts: If your breasts are mildly droopy, a saline or silicone gel implant placed behind the pectoral muscle might help create the illusion of perkiness. That’s because as the implant fills out the top of the breast, it also fills out the bottom, making it look as if the nipple has moved higher. It hasn’t. The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

In using the term perky it sounds like you would not get that look unless you had a lift along with an implant. The lollipop lift is a great lift in a case like yours. Implants alone will give a poor result.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevityIf you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

Thank you for sharing your question and photographs. In order to best approximate your goal images, it appears that you would most benefit from implant placement and a lift.  The implant will add breast volume and shape, while the lift will remove excess skin and allow for resizing and repositioning of your nipple into a higher, more youthful, position.  Hope this helps

Based on the submitted photos and description of your desired look you will need to consider a lift with implants. This can address volume, breast shape, asymmetry, and nipple position. You breast has mild ptosis bilaterally ( little more on left side). In my opinion, your breast need lift plus implant. This is mastopexy combined with augmentation. The next question is, where you want to put your implant. It depends on your choice plus tissue thickness on upper pole of the breast. And this need palpation and measurement.
