Im 20 male and my nose is to big and wide nose bridge. The side is upturned which doesn’t suit my face. I want my nose to be more defined and structured and more downturned. My nose tip is slightly slanted and my columella barely shows.

Answer: Rhinoplasty surgery may be considered for nasal narrowing. In our experience, derotating a nasal tip may be difficult, and may require a septal extension graft. Answer: Rhinoplasty Thank you for your question and sharing your photographs. Rhinoplasty can indeed achieve the goals that you have described but it would need to some preparation to make the most of it. To begin with, it would be wise to have a cone beam CT scan to make sure that the structures at the top of the bridge are normal and that it is a simple matter of wider configuration. Ultrasonic rhinoplasty can help to dramatically change the width of the bridge although in most instances it is preferable to do a moderate rather than a dramatic change. For the underside of the tip, my impression is that the tip defining points are in good position and what is needed is more curvature of the cartilages of the nasal tip with support so they do not move up again. Simple photographs are not of great value to simulate the impact of this change. 3D photographs can highlight the impact of changes in the columella and without changing the position of the nostril rim.

Answer: Large nose The nose appears good in the profile side view. From front the nasal bones, tip and nostrils appear wide. Alarplasty, Tip-plasty and Nasal osteotomies should address that. Regards Dr. J
