Could a rhinoplasty make my nose appear less bulky and more defined? (photos)

Question

Im 20 male and my nose is to big and wide nose bridge. The side is upturned which doesn’t suit my face. I want my nose to be more defined and structured and more downturned. My nose tip is slightly slanted and my columella barely shows.

Depending on the amount of nostril show you desire, a columella-plasty may also be necessary. Additionally, a downward rotation of the nasal tip can be achieved with certain surgical techniques.

It is important to keep in mind that rhinoplasty is a complex surgery, and the results may not be immediately visible as it can take several months for the swelling to fully subside. Thank you for your question and photos. You are likely a candidate for rhinoplasty to reshape the nose and create balance with your other facial features. During this surgical procedure, the nasal bridge can be narrowed, the tip refined and appropriately downturned, and the columella slightly retracted to produce the desired result. Nasal width is generally narrowed with an alar base reduction or osteotomies, or increasing the nasal tip projection to create a more narrowed appearance.

Keep in mind that rhinoplasty is one of the most complex of all plastic surgery procedures, requiring skill, experience, and artistry to produce the best possible result. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon, with expertise in rhinoplasty techniques. They will be able to discuss your aesthetic goals with you to decide on the best course of action.

In our experience,  derotating a nasal tip may be difficult, and may require a septal extension graft. Since that is one of your goals, if you decide to consult with specialists, please make sure that nasal lengthening or septal extension grafting is offered. Hope this helps! Dr Joseph

Rhinoplasty can indeed achieve the goals that you have described but it would need to some preparation to make the most of it.

To begin with, it would be wise to have a cone beam CT scan to make sure that the structures at the top of the bridge are normal and that it is a simple matter of wider configuration. Ultrasonic rhinoplasty can help to dramatically change the width of the bridge although in most instances it is preferable to do a moderate rather than a dramatic change.

For the underside of the tip, my impression is that the tip defining points are in good position and what is needed is more curvature of the cartilages of the nasal tip with support so they do not move up again.

Simple photographs are not of great value to simulate the impact of this change. 3D photographs can highlight the impact of changes in the columella and without changing the position of the nostril rim.

The nose appears good in the profile side view. From front the nasal bones, tip and nostrils appear wide. Alarplasty, Tip-plasty and Nasal osteotomies should address that.
