9 months post-op- unsatisfied with my nostrils. Is there anything I can do besides having a revision rhinoplasty?

I understand your concern. However, without a proper assessment, it would be difficult to determine what went wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Yes, it appears that further correction of the nostrils and columella may be needed. Combination of allar cartilage grafts, columella graft and additional tip refinements may be needed. I would suggest you to wait at lest few more months for result to settle and scars to mature and soften before undergoing additional surgery. In person consultation with at least two board certified plastic surgeons, experts in rhinoplasty, is recommended. Good luck.

From your photos, I note that you demonstrate relatively thick (or swollen) skin of the nose. It appears that you underwent Weir excisions at the nostril bases which are healing well. There are notch deformities at the nasal dome (perhaps due to the tightness of the Weir excisions or due to swelling). You demonstrate fullness of the nostril bases, suggesting large alar cartilage footplates (or swelling). The nasal tip is rounded and full.
Following rhinoplasty, there is a period of healing which takes at least 12-18 months. During this time there will be residual swelling which will mask the surgical result. It is too early to assess your final result. The nasal tip is notorious for taking the longest time to heal.
Premature revision is ill-advised. If the problem persists after healing, revision my be required. 15% 0f cosmetic surgeries can be improved through a secondary touch up procedure.
