Can a lip lift pull on the nose columella?

A lip lift should not pull on the columella if designed properly. You have a flat slope to your philtrum and would likely benefit from a conservative upper lip lift to restore a more concave slope to your upper lip. This should balance out your face nicely and be presentable from a frontal and profile view. Best of luck.

Your hanging columella can be corrected at the same time as the lip lift. I do a procedure where I suspend the lip inside the nostrils so that there is no pull on the columella and this is the other advantage of my technique. There are case where it is advantageous that the alar bases and columellar base be pulled down in order to make a droopy nose look more upturned. This all can be done by proper planning of the procedure. We have had many patients traveling from California  to see us in Morristown N.J. to take advantage of these refinement of the lip lift procedure.

Both your vertically long upper lip and your nostril show in profile view can likely be improved under local anesthesia in an office procedure.  Lip lift will not "pull" on columella if designed not to do so.
