Neck midline revision? (Photo)

Question

I had a face/neck lift 3 years ago. I never recovered. Neck spasm, scarring, unhappy face/neck muscles retracted my jaw. The surgery was done asymmetrically. My midline is obviously off, diagonally towards right. None of treatments work - i tried them all. I think my problem is structural. Can midline be corrected by a revision? I am scared of loosing more space and tissues with the revision if one is even possible.

It's essential to have a surgeon examine your neck to determine what exactly is happening. From the appearance, it seems there might be some tethering in your neck area. It would be valuable to assess whether this is due to muscle issues or excess skin. Depending on the findings, you might consider a full lift procedure, or in some cases, addressing the issue through the midline of the skin itself can be effective. In instances where the skin has been excessively liposuctioned, it can result in a tight appearance. To identify the best course of action, it's crucial for a surgeon to assess whether it's extra skin or if the skin and muscle are constricted together.

I would recommend a revision/secondary facelift and necklift. This would be a rather straightforward surgery to fix the issues seen in the photo. The muscle under the skin in the neck is called the platysma. You appear to have recurrent platysmal bands. Also there appears to have been over-aggressive resection of fat in between the midline. The excess skin also needs to be re-draped in a more natural fashion. I would find a plastic surgeon certified by the American Board of Plastic Surgery. I always tell prospective patients to look at the plastic surgeon's training (ideally a 6 year integrated plastic surgery residency and a 1 year aesthetic fellowship accredited by The Aesthetic Society). In addition, examine before/after photos on social media platforms. I would also look at Google or RealSelf reviews. This will help you get a comprehensive understanding of your provider.
