It appears you have a venous lake although it is difficult to confirm this diagnosis by photo. I would recommend a consult with a dermatologist to confirm that it is truly a venous lake. If it is confirmed to be a venous lake lip filler can still be performed although more caution would be needed in this focal area to avoid bleeding and bruising. If the venous lake bothers you, I would suggest laser treatment which is usually very successful. There is no problem injecting fillers into the lip in someone with a benign venous lake. If the venous lake bothers you, it can safely and easily be removed typically with 1-2 laser treatments. Occasionally, if they are stubborn or large, they can also be cut out with a minimal mark.
