Thank you for your pictures. You would benefit from jaw surgery. Specifically, you need upper jaw repositioned superiorly (upward), moving the lower jaw and chin forward. You should consult with a board certified oral & maxillofacial surgeon or plastic surgeon with Craniomaxillofacial experience.
Thank you for the photos. You indeed have a gummy smile and a long midface. You should be evaluated by an oral surgeon who does orthognathic surgery. Your facial bones need to be properly aligned in order to get a proper result. This is pretty big surgery, but often it is covered by insurance. You might get a referral from your dentist who I'm sure will know whom to send you to.
There is no question that you would benefit from more facial balance with a chin implant or sliding genioplasty if your teeth dont match up. But the other thing I see in the photo that you wanted was smaller cheeks that would be helped by reduction of your buccal fat pad. That would give you more definition and a sculpted appearance. The photographs demonstrate a very weak chin profile. Chin implants can successfully augment the chin for cosmetic purposes, while a sliding genioplasty would help with alignment of the teeth as well as cosmetic. A chin implant is inserted under local anesthesia as an outpatient procedure, while a sliding genioplasty is performed as an inpatient procedure under general anesthesia. Either procedure could be safely performed. While any inflammatory process could induce hyper pigmentation, I feel the risk is low. Both the advanced facelift and fillers can both be performed safely if desired. Additionally, good medical skin care can help long term with your melasma.
