Dear Exciting368439,my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. In my experience, best result would be anchor (T) lift with implants for upper fullness. The scars will be hidden in most bra, clothing tops.Lollipop will not allow proper removal of skin tissue and will not allow for optimal breast reshaping.An exam and consultation with a plastic surgeon is recommended to discuss your options and expectations.
According to your pictures I can surely say that vertical scar (lollipop) breast lift and implants will be perfect solution for you. Your sagging is not severe, so the anchor lifts won’t be necessary for your case.
Thanks so much for your question, The anchor (inverted-T) lift offers more extensive tissue removal and tightening, which can provide a stronger lift, especially bases on your pics which shows a lot of excess skin or significant ptosis (sagging). If you’re getting implants, the volume added can sometimes help with the appearance of lift, even if the skin isn’t as tight. But, if the skin is too loose, the lollipop lift alone might not provide the ideal outcome. The lollipop technique can carry a risk of longation of the vertical scar. Your nipples are located below your breast fold and are pointing slightly downward. Due to the significance of the sagging, a more aggressive lift than a lollipop would most likely be needed.
The plastic surgery community is somewhat split as to whether you would be better off with an anchor or lollipop lift. It’s great to get by with the lollipop to avoid the extra scars. It depends on how much confidence you have in that particular surgeon. Ask to see other cases similar to you

This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Excellent question! I believe you are right to assume that a lollipop lift is not enough. I don’t understand why a plastic surgeon would only offer a lollipop lift. I’m pretty sure he knows how to do other techniques. Every patient is different so surgeons should be a costumed to providing different techniques for different breasts. You obviously need more than just a lollipop lift. The anchor lift with its horizontal scar across the breast fold is the scar that actually lifts the most. It also helps tighten the breasts, gives it projection and helps define the breast fold. I suggest you ask for a more aggressive lift technique. In my opinion you would need an anchor lift to get better results. Lollipop lift is only good when the sagging is mild.
The short answer is yes, you can have an implant exchange without a lift. However based on the limited photo adding a larger implant alone may not give you the look you are wanting. The implants you are considering would be very heavy and may lead to lower hanging breasts. I'm not sure that...
I recommend to avoid lifting anything heavy, including your toddler, for at least 6 weeks following breast augmentation surgery. This precaution helps protect your chest muscles and ensures that your implants heal properly in place.
