Answer: How To Reduce Puffy Nipples Without Surgery? Obviously, it is difficult to make a diagnosis from a picture. That being said, it appears you have both true and pseudo gynecomastia. The pseudo gynecomastia is the fat on your chest that gives it a somewhat conical shape. The true gynecomastia is the breast tissue which is pushing out your nipple areolar complex, i.e. puffy nipples.Surgery is the only known treatment to correct this glandular problem. If you were my patient, I would liposuction the fat on your chest through an incision on the side of your chest, and then make an incision around the bottom of your areola to surgically remove your actual breast gland.
