Obviously, it is difficult to make a diagnosis from a picture.  That being said, it appears you have both true and pseudo gynecomastia.  The pseudo gynecomastia is the fat on your chest that gives it a somewhat conical shape.  The true gynecomastia is the breast tissue which is pushing out your nipple areolar complex, i.e. puffy nipples.Surgery is the only known treatment to correct this glandular problem.  If you were my patient, I would liposuction the fat on your chest through an incision on the side of your chest, and then make an incision around the bottom of your areola to surgically remove your actual breast gland.  

Surgery is the only real option to reduce long-standing gynecomastia. Discuss your options with a board certified plastic surgeon who will likely recommend liposuction and direct excision of the gland. You should obtain a nice result.

Puffy nipples is a form of gynecomastia where the breast tissue is pushing the nipples out, unfortunately there is no a treatment to reduce them without surgery. If you have had puffy nipples for more than 2 years it is most likely permanent unless you opt for surgery. Good luck.

Puffy Nipples in men can be due to various reasons that can include excess body fat, low testosterone levels, use of steroids or certain exercises. In most of the cases, changes in diet, herbal supplements and some workouts like push ups or barbell bench press can decrease puffiness. The puffy nipples is due to the underlying gland and the only way to correct it is by surgery. Hello , From  your photo alone it seems you have Gynecomastia where the gland is protruding outwardly. Unfortunately only surgery can treat gynecomastia. I suggest you consult with a board certified plastic surgery who can better examine and explain your options  in detail! Best of Luck to you
