Many years ago, I pioneered a technique specifically for patients like you termed "Reductive Augmentation". This involves an internal bra-capsulorrhaphy, mastopexy-lift, and excision of the lower pole breast tissue. You should be able to have a beautiful appearing breast with this technique. I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

first, I am not a fan of the gummy implants since they are rather firm. Second, your implants are too heavy for your own sagging tissue to support. You were sagging to start and will keep sagging with added weight. You might consider another lift and going much smaller with implants placed behind the muscle. Long term, you still may sag because you sagged initially.

Based on your posted photos, best to do an in person or virtual consultation to fully understand what is happening in your breast issue. Revision can be done and expect fees to range from $8000-$15,000. But again best of virtual concert with.

I would redo the lift with excision of some of the tissue and use mesh for support. This is the result we used to get before mesh use. The implants do not need to be replaced

Women who already have large breasts are in difficult position if they want to add implants to create an improved breast shape.
To do so, effectively requires doing a breast reduction with augmentation simultaneously.
To make an assessment, we need to know if your implants were placed below the pectoralis muscle.
I think implant placement and implants and large amounts of breast tissue have taken their toll on the breast lift.
I think the weight of your breast and the implants are pulling on your skin. Ideally, the implant is supported by the pectoralis muscle and should sit higher on your chest wall.
To make a quality assessment we need quality information.
To get a proper second opinion consultation, I suggest you schedule those in person consultations coming prepared bringing with you a complete set of proper before and after pictures and a copy of your operative report.
Augmenting and lifting a large breast is not an easy or straightforward procedure.
Sometimes these procedures need to be done as staged operations.
