Can I get implants first and then lift later to minimize potential complications?

Hi, thank you for sharing your question. You can certainly do that first. The type of implant and the location of placement is also completely up to you. After you have an evaluation by your surgeon, they will be able to guide you.

The procedure can definitely be performed at two different times, however this can be confirmed by your surgeon. They can also let you know which implant placement would be more suitable for you. Your surgeon may recommend doing the lift first.

If you are a 36 DD then you do not need implants. The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

When patients have significant deflation and ptosis, it is best to perform the lift first followed by the augmentation. This allows for optimal improvement to breast shape and nipple position before the augmentation is performed. I would not recommend patients with significant ptosis undergo the augmentation first, as this would mean a period of time where there is significant discordance between your natural breast and the implant. A single-stage procedure could also be considered, but candidacy for single-stage aug-pexy is best determined in meeting with a qualified plastic surgeon and in-person physical exam.

I would do the lift first. You may find you have plenty of breast tissue and are happy. Then, six month later, add implants behind the muscle if you want more fullness.

I have consulted on many patients that have a very similar situation to what you have described. My professional opinion would be to perform the mastopexy first. I think doing so would give you the best results and help to address some of the issues you raised. After you healed up then you go forward with the augmentation, most likely under the muscle and the implant selection would be dependent on the look/results you are trying to achieve. By far the happiest patients I have had are the ones that do the lift first followed by the augmentation.

Photos are required to accurately answer your questions. I usually do both the left (mastopexy) along with the breast augmentation. There are many other plastic surgeons who like to do this in two surgeries.
The choice of saline vs silicone and subglandular (over) vs submuscular (under the muscle) is decided at in office consultations.
