I'm 44 yo w/275 gel implants from years ago. I wanted to fill my A cup--keep small breasts, just a little fuller. The difference between band & breast measurements was 1/2 inch before surgery, now 5 inches. I'm a 34D. Had consults w/3 PS. None have experience w/small implants. and they're "not worth it" I think 100-125cc with a life would put me at A-B cup. Thoughts? I can't really afford this so don't want another size mistake. I'm 5'1", 110 lbs. breastfed 4 kids.
Answer: Small Breast Implants The biggest issue with implants in the 100 to 200 cc is their base width. They usually have a width of 8 to 9 cm across which is very small for most women. If you have a 275 in now, chances are that your existing implant pocket is too wide to accommodate a 100-125 cc implant. For your dimensions you are small framed, but your surgeon should measure your base width and choose an appropriate size based on that dimension. If you want a smaller breast, I would stick with a lower profile implant vs a moderate or high profile. If you have 275 cc in there now and significantly downsize, you may need a lift as well. Careful planning and discussion with your surgeon is most important here.

Answer: Small implants preferred At size 34 each 100 cc’s of implant corresponds to 1 cup size change.If you were initially a 34 A, 300 cc implants would take you to a 34 D, which is what occurred with 275 cc implants.If you replaced your 275 cc implants with 100 cc implants you will end up a B.However, if your skin is stretched out, you will skin resection at the same time.I recommend a new technique called Implant Exchange with Mini Ultimate Breast Lift™.Using only a circumareola incision it is possible to excise the excess skin, reshape your breast tissue creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage.Aligning the areola, breast tissue and implant over the bony prominence of the chest wall maximizes anterior projection with a minimal size implant.This will give you a youthful perky breast without over implantation.I routinely use small implants usually in the order of 185 cc’s.However, I have used implants as small as 100 cc’s.Best Wishes,Gary Horndeski, M.D.

I have placed small implants in a breast opposite a reconstructed breast to achieve symmetry in some of my breast cancer patients but you want to downsize from a larger to a smaller implant. An implant this small will need to be placed in a new pocket in the breast and you might need a breast lift. It is hard to imagine that your went from an A cup to a D cup with only 275 cc implants. It is best that you consult with a board certified plastic surgeon to give you a proper recommendation.

Answer: Small implants of 100-125 Exchanging for small implants like teh ones mentioned would probably add almost nothing to your volume. You would have to be seen in person to be evlauted. You may need a lift as well.

Answer: Trading down to a smaller size implant Yours is an unusual question. Your present 275cc implant is a routine size for plastic surgeons, and perhaps on the smaller side since many patients want 300 - 400 cc's.You must understand that trading down would mean that your breasts would be smaller. You must also realize that your breast skin has been stretched out with the present larger implant plus the result of breast feeding 4 kids. As a result, a smaller implant would "swim" in your breast and cause them to hang low (we call it a "rock in a sock"). You would probably require a concomitant breast uplift to remove the excess skin and probably to elevate a nipple which may be too low.This is a complicated procedure but it can be done.Elliot W.
