Question

So I had a breast lift in 2020 and they were nice but just too large for me and I chose a surgeon in Tampa to give me a revision/lift and that was my 4th breast augmentation. As soon as I came out of surgery I was was in more pain on my right side than I have ever been in and the pain actually never stopped. It has been a year and I have complained and sent pictures but the surgeon is adamant that the breast is lower because that is my anatomy although it has never looked this way before.

You have bottom out from the weight of the implant.  I suggest that you go smaller and use mesh for the repair.

I’m guessing your skin elasticity is not keeping up with the size and weight of your implants and keep stretching. For some individuals, it may be necessary to re-tighten the skin envelope after mastopexy surgery.
It looks like your surgeon did a good job and I don’t think your outcome is related to how the procedure was performed. Some individuals have skin that doesn’t tolerate being put on tension without eventually stretching.
 I suggest you follow up in person with your surgeon and discuss your concerns through a proper postoperative appointment. Generally do not recommend people travel long distances for elective surgical procedures.
