First surgery for lipo I was left with fat pockets and indentations on my lower belly. Second surgery/revision, my results were as expected, I was happy. However after six months, I started noticing a change in my lower stomach and back. Second surgery/revision was 360 liposuction. the "love handles" are problematic but also the indentation that runs from hip to hip. What can I do, any non surgical? Pics are from before and after second surgery and present day is black sports top and bottoms.
Answer: Revision There are some nonsurgical options you can consider but an in person evaluation is needed to determine if you are a good candidate. You can consider BodyTite which is a minimally invasive procedure, Morpheus8 or InMode options. Surgically you could consider J Plasma which helps with skin tightening. See a board certified plastic surgeon near you for a consultation.

Answer: Revision liposuction Pending a formal evaluation, you may be a good candidate for a Revision Liposuction. You need a formal evaluation by an expert in body shaping and contouring to determine what is best. The best in body contouring not only removes fat with energy devices like VASER ultrasound but also internally heats the skin with radiofrequency devices such as Renuvion (J-Plasma), InMode BodyTite, and ThermiRF to heat the undersurface of the skin and tighten it at the same time as fat removal. These combinations are called 4D HI-DEF VASER Liposuction and can only be done by a well-trained body contouring specialist. At the same time, fat should be harvested and purified, with PRP and stem cells added, and then injected into muscles, indentations and cellulite to get body shaping and contouring. For a Brazilian butt lift (BBL), we inject the fat into the buttock for contouring and lifting, but fat can also be used in areas such as the chest/breast, arms/shoulders, calves and face. Cellulite should also be treated with a combination of QWO or Cellfina, subcision, and/or fillers such as Bellafill, Renuva synthetic fat, or Sculptra. If skin is very loose before surgery, then it will need to be removed: tummy tuck/abdominoplasty for lower abdominal skin; breast lift for excess breast skin; and/or a brachioplasty for loose arm skin. Overall, you need a full assessment, but simple liposuction alone will not tighten skin and needs a combination of various methods -- fat removal, skin tightening, defining, fat grafting, skin removal -- to get the best in body contouring and body sculpting. See an expert. Best, Dr. Emer.

Answer: Liposuction results There are no nonsurgical options that are going to successfully change or improve your fat distribution. Delivering consistent quality Liposuction results is more difficult than most people realize. The number of plastic surgeons who’ve mastered this procedure and can’t deliver. Consistent quality results is pretty small. Most plastic surgeons think they’re much better at Liposuction than they actually are. Revision work is generally more complex, more difficult than primary procedures, and in my opinion, patients should be even more selective when it comes to provider selection for revision work. I recognize you’ve already had your procedures and are probably not looking to have more revision work. For those who have not had Liposuction but maybe interested I recommend Patient take the vetting process and selection of provider. Very seriously. I recommend patient begin the process by making a list of providers or clinics in your area who seem to do a lot of Liposuction work. Schedule multiple in person consultations with those providers. Avoid virtual consultations whenever possible. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar body characteristics to your own. Bring pictures of yourself as reference for any consultation or whenever you are reviewing before and after pictures. An experience provider should have no difficulty showing you the before, and after pictures of at least 50 previous patients. Being shown a handful of pre-selected images, representing only the best results of a providers career may not be sufficient to get a clear understanding of what average results look like in the hands of that provider, what your results are likely to look like or how many of these procedures they’ve actually done. There’s no correct number of consultations needed to find the right provider. The more consultations you scheduled the more likely work to find the better provider. The biggest mistake patients make is having only one consultation which more or less eliminates the ability to choose the better provider. Considering how few plastic surgeons have truly mastered this procedure and how much variation there is in among different plastic surgeons I recommend patients consider having five or six consultations before selecting a provider. Sometimes it’s simply a matter of luck. Liposuction results are on two main variables. The first is the patient candidacy for the procedure in the second is provider selection. Not everybody seeking Liposuction is a good candidate for the procedure. Individuals who are excellent candidates have the potential of having excellent results if they find sufficiently skilled provider. They can also be left disfigured, in the hands of a provider, lacking skill and experience.A patient who is not a good candidate, for Liposuction does not have the potential of having quality results regardless of who does the procedure. Understanding your own candidacy for the procedure and provider selection are the two most important variables, and I suggest patients focus their efforts on those two variables. In the end, when people have concerns regarding the outcome, the problem almost always comes down to patient candidacy and provider selection. In regards to surgical options I suggest you save your money and don’t listen to the hype. Realistically, the only way to change your fat distribution in a meaningful way is with liposuction and or fat transfer. Best, Mats Hagstrom, MD
