I am 55 and I’m not happy with my sagging lids. I am ok with the lower eyes for now but I know within a few years I will not be do yo hereditary reasons. Many family member develop bags under their eyes. Would I be better off waiting to get my lids done knowing eventually I will need my lowers done as well?
Answer: Upper Blepharoplasty Upper eyelids are fine to have done without the lower eyelids and the surgeries are completely different. I would recommend taking care of the upper eyelids sooner so you have more time to enjoy them! The surgeries are different enough that I routinely have patients just having upper eyelid surgery to stay awake for the procedure whereas just about every patient having lower eyelid surgery I have undergo anesthesia. During the surgery a excess skin and fat will be removed, and your crease will be formed and set at a bit higher. In regards to your lower lids, nonsurgical options you could consider would be dermal filler into the area for a better eyelid to cheek transition. If you want the best idea of what is possible for your eyes, do your due diligence, find an experienced surgeon, and have him/her evaluate you in person and discuss your options. #drdonyoo

If you are "ok" (they aren't bothering you) with your lower eyelids, then it would be most appropriate to leave them alone for now. You don't appear to have much excess fat (or lower eyelid puffiness) in the lower lids. There is, however, at least a mild to moderate amount of excess skin in the lower eyelid that can be improved with a conservative lower eyelid blepharoplasty (sparing the fat removal part and focusing only on skin removal). The most commonly used lower eyelid blepharoplasty technique for this indication would be either a pinch skin excision, gentle fractionated CO2 laser resurfacing, or a conservative combination of lower lid pinch with fractionated CO2 laser. From a surgeons perspective it's really down to the patient's choice whether they have the upper and lower us down together or at separate times.
