I had 2 regular rhinoplasties done by the same surgeon. The tip grew back in profile view both times (it looks good from the front) Now i’m thinking out ultrasonic rhinoplasty it is said to cause less trauma and thus it might also trigger less cartilage formation?
Answer: Ultrasonic rhinoplasty or any type of rhinoplasty procedure does not create the formation of more or excessive cartilage. Without photos, it's difficult to understand exactly what your concerns are because for example, if you tend to have a bulbous tip, or if you are of ethnic origin the skin may be thicker thus being heavier at the tip. Meaning it would be difficult to refine the tip of the nose without the support of some cartilage. Often, grafts are needed to refine or define the tip of the nose in these types of cases. Ultrasonic rhinoplasty does not have an influence and is not used to create a defined nasal tip. Ultrasonic rhinoplasty device (Piezoelectric) selectively works on the bone, so ultrasonic rhinoplasty would not affect the concerns you are having with your nasal tip. If the tip is not well defined it is because you develop scar tissue or you need more support, very likely that you will need to use grafts in different positions or perhaps an extension graft and if it is an ethnic nose or a nose that does not have great strength in its nasal cartilages it is likely that it is necessary to use rib cartilage grafts to help support and refine the tip. Nothing could stimulate more cartilage formation, perhaps still there are some residual cartilage left on your nasal tip. Hope this helps, and I wish you the best.

Answer: Cartilage does not grow back after rhinoplasty surgery. Consider re-asking your question with photographs. Before and after photos may be helpful. Cartilage that is trimmed or removed does not grow back after rhinoplasty surgery. The same goes for nasal bone that is removed, such as a dorsal hump. Based on your description it sounds like you need a nasal tip deprojection during your revision which has no connection at all to ultrasonic rhinoplasty. Here is some general advice in selecting a surgeon for your revision: I highly recommend that my patients focus much more on real longterm before and after photographs rather than 3D imaging. I encounter so many patients in my practice who see me for revision rhinoplasty who previously had surgery elsewhere who feel like they were mislead into surgery by surgeons who relied heavily on 3D imaging without adequate real before and after pictures to back it up. Photoshop is easy but real surgery is very different. I always recommend that you carefully evaluate your surgeon’s online before and after gallery on their website for both quality and quantity of results. Make sure that there are hundreds of real before and after pictures which demonstrate long-term follow up results. If your surgeon is posting mostly on table results without real long-term follow up results, that is usually a major red flag. If a surgeon is posting mostly just splint removal day videos without longterm follow-up pictures, that can also be a red flag. Long-term results are much more meaningful than on table results or 1 week post-op results.
