Hello, I'm in the process of trying to get approved by my insurance for breast reduction surgery. I have been going to PT for the past two months and it's not helping me. I have severe neck, back and shoulder pain. I'm nervous to ask my PT for a letter to help me out with my surgery. What documentation do I need from my PT that would explain to my insurance that the surgery would be the best option for me? I'm 138lbs 5' 3" and I wear a 32HH. How should I go about asking my PT?
