Any suggestions on bulge of fat/muscle in front of ear? (photos)

Question

I have a strange bulging of fat and muscle on my right side that completely blocks my ear from the front. Bigger when jaw clenched and ear relaxed (if I perk my ear up it decreases the bulge). My masseter muscle isn't excessively large. I have had 3 Le-Fort surgeries but I vaguely remember this problem before, so am doubtful it is related. I have no idea what is causing this or how to treat it. All opinions welcome, thank you for your time! :)

Thank you for your pictures and question. It is impossible to tell from the picture what could be causing this swelling. It could be related to muscle hypertrophy, glandular swelling, or something else related to your multiple facial surgeries. I would suggest seeing the surgeon who performed your LeFort surgeries to start the workup for this problem. It will certainly require an in-person exam and possilby additional imaging such as a CT or MRI to fully delineate the problem. Best of luck!
