Thanks for the question, I would have a chat with your hematologist and discuss the risk profile of coming off of the meds for a short time. The few times this has come up for me I was not able to justify the risk of the procedure compared to the risk of a blood clot with the medication coming off. However, I always am willing to see the patient and discuss with the hematologist

Pre-operative clearance will be required including and especially your blood counts and your clotting studies.
I don’t think the risk/benefit is in your favor but I would talk to your hematologist as they are probably in the best position to analyze your true risk and can give you the best advice. For me, I would not operate on a patient in your situation unfortunately.
You would need a written clearance fir surgery from your treating MD to be safe and clear if you can tolerate an elective operation...
