The occipital rolls and their associated deep creases can only be improved by excision. I would do the lower deep one first. While it creates a sine line scar at least the roll will largely be gone and the flatter scar is better than the deeper groove.

Pain is normally felt after surgery. It might persist for a few weeks to a few months but would resolve on its own. Pain medication prescribed by your board-certified plastic surgeon would help ease the discomfort.
