African American Rhinoplasty. What are my options? (Photo)

Question

I don't like the width of my nose when I smile, and the bulbous tip frontal projection. What are my options, I would prefer not to have my nose broken. I also have thick skin, I do like the side profile of my nose. The last picture is a digital frontal of my ideal nose.

It's all about architecture or the lack there of. The structure between your nostrils, the columella cannot hold the tip of your nose up because of weak cartilage. The tip, which is shaped like an arch gets pulled closer to the face and falls some. This is called tip ptosis and causes the tip to round out. Until your tip is supported with a columella strut, surgically the nasal tip will fall. This only gets worse with age.

Hi @mhchris20! Thank you for sending you photos--you would be a great candidate for a rhinoplasty.  You can narrow the bridge and refine the tip. A rhinoplasty in my opinion is one of the most difficult procedures to perform. The skill, artistry, and technique of the surgeon will determine the outcome. In my practice, I utilize piezosurgery. Piezosurgery is a more gentle technique that allows for greater precision. Less bruising and swelling is common with Piezosurgery. In a typical rhinoplasty, a hammer and chisel technique is performed leading to more inadvertent fractures. Best of luck, Dr. Nima!

From your photos, it appears you are a very good candidate for rhinoplasty. This procedure could narrow the bridge and refine the tip. By adding alarplasty, you can also reduce the size of your nostrils. You are a good candidate for rhinoplasty. You have a small bumpOn the bridge which can be removed. The entire nose can be made narrower on the tip and bridge. Your nostrils can be reduced with a procedure called alar wedge excisions.

You can have the lower half of the nose (tip and nostrils) addressed without breaking the nasal bones. Tip-plasty can address the bulbous tip either through closed or open approach to make it narrow and refined.
Alarplasty is an elegant technique to remove some of the nostrils where it attaches to the cheek area. The scars are well hidden. 
  
Both of these can be done under local anesthesia.

A closed rhinoplasty approach can accomplish narrowing the bridge line, shaving down the dorsal hump  and narrowing the nasal tip cartilages with all the incisions placed on the inside of the nose. Thick skin in the tip of the nose will prevent refinement in that area. An alar-plasty accomplishes narrowing wide nostrils. This can be performed at the same time with a closed rhinoplasty under general anesthesia, or it can be performed under local anesthesia as a standalone procedure.
