Please Advise. Sculptra and Kenlog nightmare. (Photo)

Question

Surgeon placed sculptura in neck last July and nodules formed within weeks. He then over treated with Kenalog. Now I have atrophy from kenalog and discoloration. My chief complaint was neck rings and now I have a disfigurement. 2nd opinions say surgically remove nodule but I think a scar would make this even worse. Please tell me what I should do or where to go to have this corrected. I’m just sick about it and need an expert’s skills. Thanks!

I’m sorry to hear about the unwanted side effects of your Sculptra treatment. Kenalog has beenshown to counteract the effects of Sculptra but has not been thoroughly researched and can beunpredictable. Surgery would certainly be the main recommendation for removing the nodule,although it will leave behind a scar, as you mentioned.If you do move forward with surgical removal, then laser skin resurfacing treatments may beused to improve your scar and overall skin texture. Continue to consult with board-certifiedplastic surgeons to explore all of your options before moving forward. Hope this helps!

Sorry to hear about your experience with Sculptra in the neck. Sometimes Sculptra nodules can be improved with Kenalog or 5-FU, however, this is inconsistent. The only other options would be to surgically remove the nodule which will leave a scar or let the product naturally wear down which could take years. Any scarring from surgical removal can be improved with lasers, etc. I don't use Sculptra for horizontal neck lines. These are better treated with fillers, but require an advanced injection technique regardless. Hope this helps answer your question. Best wishes.

Agreed. You do have a few options available- steroid with CO2 fractional under drip occlusion can be performed. The definitive management would be excision, then laser to improve the scar line. All the best, Dr Davin S. Lim. Brisbane. Australia.
