I’m planning to have a nose tip reduction. I have medium-thick skin and a wide nose, and I only want work done on my nose tip. During my consultation, the surgeon mentioned he would use a cephalic trim technique for tip refinement. He also explained that due to swelling, my nose might look wide and similar to my old nose until around 7 or 8 months after the surgery. He said it could take that long for the swelling to go down and for the refined look to appear. Is this accurate?

Answer: I believe your surgeon is being conservative. Since you appear to have relatively thick nasal skin, along with a cephalic trim, a tip graft may be considered for achieving narrowing and definition. I don't believe it would take 7 to 8 months to start to see nasal tip narrowing, in our experience. If in doubt, get a second opinion. Thank you for this question and I wish you well moving forward. Of course, I haven't had the chance to *feel* your nose, but there are plenty of clues in the photo that you don't have thick skin at all. Then, I doubt very much that a "cephalic strip" would succeed in narrowing your tip. It's in all the surgery books, but it rarely works very well. And finally, if the surgeon has good success in narrowing a tip like yours, you should see a significant narrowing on the day that the splint is removed, seven days after surgery. Maybe re-evaluate your surgeon. Have you seen his before and after photos for patients with wide tips?
