Question

I am not satisfied with the result of the surgery. The breast is not filled in at the bottom and the skin is loose. My doctor said that a lift was not necessary, but now he is suggesting a lift to correct it. Are there any other possible options to fix this? Before the surgery, this was a 375 ml Motiva Ergonomix implant planned to be placed using the dual plane technique.

Hello, I completely understand your concern. you have to wait for atleast 2 months for the edema to settle down and appreciate the results. Also with the gravity the breast may sag down and the lower can be better seen. However, if after 3-4 months you still have same concern, you may consider fat grafting for the same. It all depends on how long ago the surgery was.  If recent then you might need to do some massages to have the pockets loosen and the implants fall into place.  If it has been a while that might not happen and if you have a capsular contracture, then a capsulotomy might solve the problem.

   Thanks for posting your before and after pictures. From observing your before pictures you needed a combined procedure of breasts augmentation with mini lift (donut, circum areola) . Currently , to obtain  attractive and youthful looking breasts you still require mini lift with additional inferior capsulotomy (scar release) .
   Always consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after photos of redo breasts augmentation to make sure that they are numerous, consistent and attractive with nice cleavage, perky, symmetrical and natural looking.

You have a very nice aesthetic result from the primary augmentation. After reviewing your post-op picture, I understand your concern related to the lax skin at the lower pole of the breast. Based on the photo, I don't see a lot of excess in the horizontal direction, so I think you could likely obtain your desired results with either a Concentric lift (incision around the areola) or a Lollipop lift (incision around the areola and vertically down to the inframammary fold).
