I don't like how heavy my lower face / jawline /jowel area is.Would buccal fat removal help? Or any other recommendations?? I'm 36 and have a slim figure at 5'7" and 120lbs
Answer: Treatment for jowling I can see your concerns in the frontal photograph. I don't think that buccal fat pad reduction is indicated. For temporary improvement, you might consider fillers placed in the jowl area to provide a smooth, continuous jawline from chin to angle of the jaw. A more permanent solution might be some facial liposuction and a short incision facelift to re-drape the soft tissue along the jawline. Lastly, botox along the lower portion of the masseter muscle might diminish the lower width of your face and provide a more V-line shape.

Answer: Lower Facial Contouring The heaviness in your lower face is not going to be improved by a buccal lipectomy as that effect is much higher than where you want the area reduced and will either make no change or may actually make it look worse. (as the area above the heaviness becomes thinner) At the least subcutaneous defatting of the perioral and lateral face by very small cannula liposuction will provide some modest benefit. But it also appears that you may have some masseter muscle fullness which is making your jawline fuller/thicker. That can be verified by seeing how the muscle bulges when you bite down and would initially be treated by Botox injects to see the effect of what thinning the muscle thickness will accomplish on your jawline contour in the front view.

Consider placement of fillers in the pre-jowl sulcus for a temporary improvement, or placement of a pre-Jowl style chin implant for a permanent augmentation. You're not ready for a mini facelift. Chin implants can be placed under local anesthesia as an outpatient procedure.
