Can a second buccal fat removal be performed if the patient isn’t happy with the results?

Not everybody has the same amount of fat in the buccal area. For that reason those with small collection of fat will not see such a significant difference like those with very bulky buccal fat compartment. Only your surgeon may know that and give you information how much she/he has removed and what was available? So, if most of fat was already removed, there may be no benefit from additional removal. Depending of the circumstances and policy of the office, patient will pay certain fees for revision surgery regardless of the procedure. These are issues that only your surgeon and management team can address.

Technically it is possible to get a second buccal fat removal if a patient isn't happy with the results, but the desired improvement cannot be guaranteed. The reasons are multiple and they are:Maybe there aren't  residual buccal fat pads left Maybe there is be excessive post-surgical scarring that doesn't allow to redo the surgery in a safe mannerIf the BMI is elevated (weight for height), no matter how much buccal fat pad is removed, the results will be masked by the excessive body weight. The cost of undergoing a second buccal fat removal or "retouch surgery" depends on the policies of the practice of the surgeon. Some surgeons charge an specific amount for any surgical retouch, others waive the surgeons fee but charges the operating room and anesthesia cost. If the redo is performed by a different surgeon ( who didn't do the initial surgery) it is obvious that he/she will charge the full amount  regularly charged for the procedure. 

If would be very uncommon for most surgeons doing a buccal lipectomy to leave much behind. I have re-explored numerous prior buccal lipectomy sites in which the patient wanted more or thought that not enough was initially removed....only to find a negligible or no amount of further fat to remove. While a secondary buccal lipectomy would be very straight forward to do the odds are that not much is left to remove. It is also very possible that where you think the buccal fat pads extend to or the effect you think their removal would create is somewhat different than where the buccal fat pad is located. As a general rule if you draw a line from the mouth corner to the attachment of the earlobe to the face, a buccal lipectomy only reduces the fullness of the cheeks that lies above that line. What lies below it is not the buccal fat pad but the perioral mounds which is a different layer of fat in the cheeks.
