Hi Jtomalley,Thank you for sharing your question with us,A Consultation with your chosen surgeon, will give you the opportunity to discuss your requirements and have an examination of the areas of concern. The facelift I perform is under a local anaesthetic which means a significantly reduced downtime.

It's best to begin by consulting with a surgeon in-person. The presence of banding down the front of the #neck, and, amount of excess skin and condition of your skin are among some factors which determine what treatments will best rejuvenate your neck . During your initial examination, your chin will be evaluated along with the neck and jawline. A weak #chin implies a lack of chin support for the skin along the neck line. In younger individuals a chin implant alone is usually all that is needed to improve the appearance of the neck. Subsequently, it will also improve the overall #balance of the face and can create the illusion of a smaller nose. As a stand-alone procedure, chin augmentation can be performed in the office under local #anesthesia. Unfortunately, the noninvasive skin tightening techniques that are currently available have produced less than optimal results on the neck. It is my opinion that the only way to get dramatic improvement in loose and crepey neck skin is to go for a neck lift, which will also tighten the underlying platysma muscle. A neck lift will give you a dramatic improvement in your jawline and neck and should last for up to five to eight years.

A neck lift would help re-tighten the skin under the neck. For patients who'd also benefit from a facelift, these two procedures are commonly performed at the same time. An incision which runs in front of the ear, behind the ear, and along the posterior hairline allows the surgeon to remove the excess skin from the face and neck. This tightens the skin throughout the neck to improve the crepey appearance. A properly performed face/neck lift would not make you look "pulled," and instead should make you look very natural.

 You're creepy neck has both vertical axis skin as well as horizontal axis skin and small bands. The best treatment for this is a neck lift with excision of the small bands in the central neck. See an experienced   facelift surgeon to advise you. Minimally invasive procedures will give such very slight results for you that I do not think they are worth it.  I have seen many patients who have tried minimally invasive techniques and feel it is a waste of money and that they should've saved their money for a neck lift or facelift.

The worst way to tighten a neck where little extra fat is present is to attempt is a radio frequency tightening device or laser liposuction for a crepey neck.   This can defat the neck and speed up the timetable for a formal face and neck lift.We often recommend radio frequency tightening for patients who have excess fat in their necks.  Remember, the fat in the neck is the only thing hiding those glands, the stringy platysma muscle and the trachea; defatting the neck can make all of these more visible!However there are laser based external tightening devices that don't melt the lower subcutaneous fat.A skilled and artistic surgeon who offers both minimally invasive and formal surgeries will be able to advise you on whether the skin is the issue, the platysma muscle, the jowl or deeper tissues, or possibly even the glands.There are many types of neck lifts, lower facelifts, neck only lifts, etc.  that you might want to read about.  Surgeons do not agree on what the terminology is!  With so many different methods, all of which sound convincing, it's doubly important to take a deep breath, and listen and watch very carefully during the consultation.  With a little time and a few consultations with board certified plastic surgeons with great reputations, the decision will become much easier than you might imagine!For patients wanting the very best and most definite option, a cleverly performed face and neck lift can make all the difference.If these are being performed, then this is the time to contemplate cheek elevation, the all important eye shape preservation (to prevent downward pull of the corners of the eye) and to prevent the appearance of having had plastic surgery.  Ancillary procedures, when performed subtly, can make produce a very natural, believable youthful appearance.

First of all congratulations on your lifestyle changes! The unfortunate downside to losing weight, is that when you "deflate the balloon" with the weight loss, your skin elasticity determines how much the skin shrinks. In our 20's this is not a problem, but after 40, there is less elasticity and we are left with the crepey skin. Non-surgical treatments are either temporary, as with neuromodulators, or are less effective than most patients desire, so a neck lift with re-draping the skin and removing any excess is the most effective treatment.
