It looks as if your hematoma or seroma healed with scarring.  This is very difficult to treat without another revision surgery (which would ultimately be my recommendation).  however, you can try steroid injections (+/- 5FU) , RF treatments like velashape or venus legacy, ultrasound treatments like Ultherapy, and/or  botox in this area.  However, most instances, a revision surgery is indicated.  Also it looks as if liposuction was done and there is irregularity/divot under your chin that should be fixed as well at the same time.  Best, Dr. Emer.

You are only 4 months post op so I am hopeful that steroid injections can still be beneficial in breaking down the scar tissue in your neck. Unfortunately, there is a possibility that the scar tissue will remain and you will need a neck revision surgery. Good luck and I hope for the best.

Your photo shows significant scar formation which is not uncommon after blood pooling. You can try repeated steriod injections and massage but if significant improvement doesn't occur after a few more months, surgical revision may be your only option.  Your Board Certified Plastic Surgeon can help you with this decision and you should get a nice result.
