I have 325cc saline implants under the muscle for 17 years. I dont have any contractures.

Thank you for sharing your question.  Costs for any surgery revolve around the procedures to be performed as well as the anticipated time of the procedure for anesthesia and facility costs.  As a result, without knowing your current breast appearance it is difficult to offer a price range. I would see a series of ASPS board certified plastic surgeons in your area to discuss.

Thank you for the question. Keep in mind, that there are a variety of different ways to remove breast implants and the en bloc removal is not always necessary or indicated. This procedure does take longer to perform (and is associated with a longer incision);  costs of surgery are generally greater when patients undergo the en bloc removal.

Generally speaking (in my opinion), unless the breast implant capsules have thickened (and/or are otherwise symptomatic), are associated with the ruptured silicone gel breast implants, or if the patient has concerns about "medical conditions" related to the breast implants, capsulectomy is not universally necessary. For these patients, en bloc removal of breast implants is a good procedure.

On the contrary, capsulectomy can expose patients to additional risks, such as bleeding, size loss, contour irregularities and other serious complications. In other words, any maneuver performed during surgery exposes patients to additional risk (morbidity). For example, attempting to remove very thin capsule densely adherent to the patient's rib cage may expose the patient to significant bleeding and/or entrance into the thoracic cavity.
 
When it comes to costs of surgery, there may be many “variables” involved. The specific surgeon's credentials/experience level (or lack thereof), the specific geographic area you are looking at, the different operations available, the location of the procedure (quality of surgery facility), type/quality of anesthesia provider… are some of the potential variables involved. You will find some average costs of surgery on this website and on individual plastic surgeons' websites as well.

In regards to outcome of the planned procedure, more important than costs of the procedures will be careful selection of your plastic surgeon. I would suggest starting with the American Society of Plastic Surgery and/or the Aesthetic Society of Plastic Surgery to obtain a list of well experienced board-certified plastic surgeons.

Then, I would suggest you visit a few surgeons whose practices concentrate on aesthetic surgery. ***Ask to SEE lots of examples of their work AND preferably speak/see patients who have had similar procedures done.

Be very careful with your decision-making. You will find, while doing your due diligence, that there are many different “specialties” who will offer their services to you; again, I strongly recommend you concentrate on surgeons certified by the American Board of Plastic Surgery.

I would also suggest that you do not base your selection of plastic surgeon primarily on costs of surgery; keep in mind that further revisionary surgery may be more costly in many ways.

The cost of surgery has several components.  They are largely driven by time in the operating room which will be determined after a surgical plan is in place.  Due to the length of time you've had your implants it may be wise to have the capsules removed as well as the implants. Cost include surgeon's fees, anesthesia charges, and facility cost.   To get the most accurate price quote visit a board certified plastic surgeon in your area for a consultation.

If you don't have any contracture, there is no reason to remove the capsule itself.  The capsule can be used to perform certain suture techniques to create a better breast shape.  Fat transfer can certainly be performed at the same time as implant removal and is quite common in our practice.  Cost does vary based on every patient is different.
