Question

I have a high forehead with a very masculine looking hairline. I would like to have my hairline lowered a bit and rounded out to a more feminine shape. Am I a good candidate for hairline lowering / forehead reduction? I may also be interested in a brow lift in conjunction with that. Let me know your thoughts please.

Hello. Yes, you are a suitable candidate for forehead reduction surgery. It can be reduced up to a maximum of 3 centimetres. Since you are light-skinned, your scar will be at the minimum level. Most patients with a hairline like yours would choose a hairline lowering followed by a hair transplant to round the hairline 3 months later.  A hairline lowering can not round the hairline in someone with temporal recessions like you have.

This is not a commonly performed procedure in my office, but the few patients that I have treated have been very happy. The forehead certainly can be reduced and the hairline brought down with a large excision of skin. The negatives of course are the long scar and changes in sensation and temporary itching as the skin heals. The surgery is relatively simple and the recovery is not difficult.

The question is not whether you would be a good candidate for hairline lowering as you would be, but whether it can be done alone or requires a first stage scalp expansion. That depends on two factors, how much natural scalp elasticity you have and where you want the new position of the hairline to be.

Based on your photos you appear to be an excellent candidate:(1) anterior hairline is thick/dense with follicles(2) lowering your hairline with dramatically rebalance the features of your face, in other words more focus on the face as a whole and not on the forehead(3) I can only see your left "widow's peak" and that can be reduced/closed to round your frontal hairline. 
I do not, however,  see a need for a browlift--your eyebrows are at an aesthetic height and shape and your upper eyelids are well defined.
The surgical technique determines the aesthetic outcome from hairline position, to its shape, and invisibility of the hairline scar.
