A year post rhino/septo. Before had a hump &defined nose. I loved my front &wanted that to stay the same. Wanted the bump taken down a bit &ended up with a smashed/flat undefined crooked nose. My lip also doesn't lift up like it did before. My nostrils look huge &flared, my tip has been overly deprojected & is turned down/low. My nose looks flat &long. Also weird lines by my nostrils &bridge. Do I need augmentation? Any revision advice on how to fix
Answer: Revision You may benefit from surgical revision to shorten your nasal length, rotate the tip higher, and produce a supra tip break. Cartilage grafts could be placed to address asymmetry of your alae. Consider consultation with surgeons with significant experience performing revisions and a practice that prominently features rhinoplasty. Best of luck.

Answer: Revision rhinoplasty to lift tip and create more natural shape Thanks for the question. Of course it would be helpful to assess your nose in person, but based on the photos, it seems spreader grafts combined with osteotomies may be appropriate to straighten your bridge. To lift, straighten, and support your tip, my preference is to use a caudal septal extension graft and septocolumellar suture. Your lateral crura might need to be strengthened with strut grafts. If septal cartilage is insufficient, ear or rib cartilage are options. I'd suggest consulting with board-certified surgeons who specialize in rhinoplasty. Overall, your revision doesn't seem to be super complicated, so keep hope alive!Best regards,Dr. Mehta

Answer: Revision Rhinoplasty Candidate Rhinoplasty is an intricate facial plastic surgery procedure. It not only effects the functionality of your nose but also the form. There are often an assortment of reasons that patients are unhappy with their initial nose job. In your case, you want to correct your nostrils and nasal tip. Revision rhinoplasty is an option to address these issues of concern. When performing revision rhinoplasty or a secondary nose job, it may be necessary to employ an open rhinoplasty surgical technique which entails placing an incision across the columella (skin between the nostrils). This surgical technique allows for a facial plastic surgeon to have improved clarity. Subsequently, for my patients this allows me to better see and approach the problem. The best piece of advice I can give you is to seek out a revision rhinoplasty specialist that is board certified in facial plastic surgery. A rhinoplasty done in the hands of an inexperienced surgery takes longer and you may be able to speak better to this than most others, often leaves you with an undesirable result. Do your research and go on a few consultations. The more unified your vision is with your surgeon, the better your end result will be. I wish you the best on your journey toward fixing your nose. Please feel free to contact our New York City office with any questions you might have. I am also including a website link with more detailed information about Revision Rhinoplasty as well as a video that discusses the common reasons for having a secondary nose surgery. Sincerely, Philip J. Miller, MD, FACS Double Board Certified Facial Plastic Surgeon
