52 yo patient was found to have right breast ductal carcinoma in situ. Lumpectomy was carried out with very close margins. Because of a strong family history, the patient wished to have bilateral mastectomy. Immediate breast reconstruction was performed with muscle sparing free microvascular TRAM flaps. Six months later nipple reconstruction was achieved.
