Dr. Johnny Franco is a board-certified plastic surgeon in Austin, Texas. Known locally as Austin Plastic Surgeon, Dr. Franco provides a complete range of cosmetic procedures and plastic surgery, including facial rejuvenation procedures like dermal fillers, lip and jawline augmentation, breast augmentation, and Brazilian Butt Lifts (BBL), his primary specialty. “BBL is my number-one procedure,” says Dr. Franco. “I perform up to 240 of them a year.” Because of this expertise, he trains other surgeons at the American Society of Aesthetic Plastic Surgery (ASAPS) annual meeting and online how to safely perform it and achieve superior results. After graduating from Beloit College with a B.S. in Biochemistry, Dr. Franco received his MD from the University of Texas Medical Branch at Galveston. He completed his residency in plastic surgery at the Saint Louis University School of Medicine, serving as chief resident, before completing fellowships in head and neck microsurgery at Chan Gung Hospital in Taiwan, breast reconstruction and fat transfer at Gent University Hospital in Belgium, and aesthetic plastic surgery at Miami Plastic Surgery. Dr. Franco is board-certified by the American Board of Plastic Surgery, a member of ASAPS and the American Society of Plastic Surgeons (ASPS), and a fellow of the American College of Surgeons (FACS). Dr. Franco was voted the best plastic/cosmetic surgeon in Austin by the Austin American Statesman in 2018 and 2019. Over 30% of his patients travel to Austin from out of state, and his practice offers a “fly-in” program. He’s also bilingual, fluently speaking English and Spanish. RealSelf Q&A with Dr. Johnny Franco RS: What inspired you to be a surgeon? JF: I was a nerdy guy who loved science. I grew up on a small pecan orchard doing manual work on the farm, and I knew I wanted to do something with my hands. Surgery was the perfect balance between science and manual labor. RS: How did you decide to specialize in aesthetic surgery? JF: It was an evolution. I did several fellowships, including one at Miami Plastic Surgery for three years. BBL was the number–one procedure I did there. When I opened my practice in Austin, there was a need for Brazilian Butt Lift surgery but no one was performing it here in Austin. It quickly became far and above the top procedure I performed. RS: What’s special about your results? JF: Perfection is something I strive for. My anesthesiologist says I’m the best liposuction person in town because I don’t get tired or bored and move on. I go at it until it’s perfect. RS: What sets your practice apart? JF: People feel really comfortable here. Patients like that they can talk to me and get an honest answer. Sometimes it’s not what they want to hear, including that they’re not a good candidate for a procedure. But those people refer so many people to me, because—in the end—they always appreciate that I was honest. Patients appreciate our upfront nature about everything. For instance, we’re in the process of listing all of our prices online—from injectables and facials to surgical procedures. We want to be as transparent with pricing as is humanly possible, to help guide patients to what’s in their budget. We also use technology to streamline the whole process. Even pre-COVID, we did virtual consults, and we offer virtual follow-ups. Patients can message with questions and we text right back. They love this aspect of my practice.

Q: If I had a breast reduction with 400g removed & want old breast size back, can I get 400cc implants?

A: Implants can help to restore volume after breast reduction, but the correlation of grams removed to implant volume isn’t always 1:1. However, you can use sizers in office to get a good idea of what size implant would be best for you.

It is important to consider carefully what your goals are regarding the implants. Often patients don’t miss the size but are looking for upper pole fullness, or cleavage. I if this is a case, often a smaller implant will be able to add that fullness without having to dramatically increase size. Another great option can be fat grafting. A consultation with a plastic surgeon will help you to decide what’s right for you. Best of luck!

A: While implant position can change after 1 year, they tend to be minimal or subtle changes. From the pictures the implants appear to be well positioned. Your main concern with the upper pole fullness may be from the profile. It may be worth considering a change to a slightly lower profile to give you less upper pole fullness. With Natrelle that would be the SSM or moderate profile. Best of luck!

A: Choosing the right size for a breast augmentation can be so very difficult. Unfortunately, the only way to change this is with revision surgery. Going down in size can be challenging. If trying to go down significantly in size additional procedures may be recommended. Other factors can impact the overall look as well such as, location of the implants (above or below the muscle), profile, saline or silicone, and if silicone type of gel. Your best bet of course is to schedule a consultation to discuss almo do this and see what your best options are moving forward. Best of luck!

A: Congratulations on the weight loss that is an amazing achievement. Combining an abdominoplasty with liposuction can help to achieve a small waist, it can also help to provide more shape posteriorly. A second option is to perform a 360 lift, basically a tummy tuck that goes all the way around. This is a bigger recovery, but often provides a more dramatic result. From the photos you have provided, it appears you have loose skin on your back, so this could be of benefit. Lastly, it does appear you would benefit from a breast lift, rather than implants alone. The deflation and downward pointing nipples will result in a “waterfall” deformity with implants alone. It will look like the nipple is falling off, or hanging off the implant. A second option would be to perform fat grafting at the time of mastopexy. This can avoid needing an implant, but provides a slightly different look than an implant can achieve. Hope this has helped. A: Great question. Often patient’s are looking to combine procedures to reduce recovery time. But you raise an interesting question, would one doctor be able to achieve the results you want in each of these areas? The short answer is yes, many or all of these can be combined into one procedure. A more common reason for dividing procedures into multiple surgery’s is for patient safety. Occasionally, the combination of procedures will take too long and splitting them up into more than one surgery is safer for the patient.

Choosing the right surgeon for you is important. And look at their results to ensure they match your goals. A surgeon with experience in massive weight loss and body contouring should be able to comfortably address these issues as they are commonly affected areas with massive weight loss. It would be best to obtain at least 2 consults. Hope that information helps.
