Hi, I am 22 I’m 5’4 and weigh about 130 pounds. I had a bbl in maryland last year and now I am looking for more projection, my dr putt 700cc in cheek. My question is can I get another bbl and have dramatic change or should I go with implants? I don’t want to spend more money and not get the dramatic project.

Answer: Second BBL A second BBL may be a good option if you really want to have a larger bottom. Projection can sometimes be inherently limited in certain parts of the buttocks because of connective tissue that anchors the skin to underlying muscle fascia. This can be particularly true for the inner lower part of the buttocks. There are inherent limitations in what can be achieved with a single round of fat transfer. Typically secondary BBL‘s or second rounds of fat transfer are often more successful than the first because the area has more host tissue to support grafted fat from the previous graft. Implants may be an option but that procedure has a long list of complications and potentially undesirable side effects. The majority of plastic surgeons in the United States do not offer gluteal implants and there is a good reason why they don’t. To me your bottom looks excellent, is proportional and sometimes stopping while you’re ahead is a good idea. To judge the outcome of a procedure we really need to see proper before and after pictures. If you don’t have your before and after pictures then ask your surgeon to forward the ones they took. If you opt for gluteal implants then I suggest finding a plastic surgeon who does that procedure on a regular basis and puts the implant intramuscularly. It seems that the key to getting good outcomes gluteal implants is to use appropriate size implants that properly fit inside the gluteus muscle and placing it in the muscle. Implants placed above the gluteus muscle often become visible and can shift position. Best, Mats Hagstrom MD
