When a person smiles, the muscles of the face move the cheek toward the eyes, and this normally causes the cheeks to appear fuller. This is completely normal. It is difficult to fully evaluate your face with just the one picture you provided. A picture of your face when you are not smiling would be helpful or an examination would even provide more information. If there are any minor differences between the cheeks of each side, this can usually be improved with injectable fillers.
Hi, I have performed many facial shaping procedures over the past 30 years. While non smiling photos from the front and side are best for evaluating the cheeks and face, the bunching that you describe is caused by excess fat in front and below the cheeks. Smiling pulls this fat up and on to the cheeks. It's not uncommon to have more fat on on side of the face. Following the beauty principles outlined in my book on face and body beauty, women look the most feminine, youthful and attractive with heart shaped faces. Heart shaped faces have cheeks that are full and round in the front. Cheek augmentation with a dermal filler or using cheek implants for a permanent enhancement will create full, round cheeks that will feminize the entire face. This would be combined with liposuction to reduce the unwanted fat for further shaping of the cheeks.Hope this helps.

Facial sculpting and contouring can be created with well-placed fillers (Voluma, Countour, Radiesse, Bellafill) and Sculptra, which would give improvement in facial balance. Fat or implants are alternatives. I recommend getting a formal evaluation with a facial sculpting expert. Best, Dr. Emer
