Plastic surgery scars require attention after surgery so that they remain cosmetically-appealing and barely noticeable. In our office, we use a protocol for scars that minimizes their activity in the 6-8 weeks after surgery. If you have late plastic surgery scars, you may also require laser and RF therapy to help reduce the longterm effects of the scarring. Keloids, hypertrophic scars, widened scars, red scars, and hyperpigmented scars all require scar modulation with topical creams followed by lasers and RF therapy. Topical skincare should be directed to a specific need, whether for pigmentation or hypertrophy and our office can guide you through the process to obtain the correct Plato’s Scar cream or Melarase cream for hypertrophy and discoloration. In terms of lasers, we would employ the vbeam laser for redness, Venus Viva RF, PRP microneedling, chemical peels, and Morpheus 8, fractional erbium, or fractional co2. I recommend consulting with your Plastic Surgeon and getting an exam and evaluation. The scar may be helped with a scar revision but without an evaluation, it is difficult to say for sure what is going on.
A Year and Half Post Breast Augmentation - One Scar is Still Very Painful. This is very unusual. You may want to attempt a course of scar desensitization. It could be a neuroma that requres surgery.
At this point in your recovery, it is uncommon to have persistent scar pain. Something is likely irritation the scar either internally or externally. I recommend a visit to your plastic surgeon to evaluate the scar. The treatment can range from injections to a total scar revision.
The discomfort that you are describing can be related to a scar that has not healed ideally. That is, it may be red, thickened and elevated at least slightly. If this fairly well describes the scar on your left breast, then it often can be easily treated by cutting it our and reclosing it simply using local anesthesia. Rarely, a small amount of steroid medicine injected into the area can be helpful.
If the scar is causing you problems you may wan to see someone regarding scar care or possibly revision.

Ask your surgeon to evaluate you. Some times a painful scar can be improved with a simple in office revision.
The scar should not be that tender 18 months after surgery. I would suggest that you discuss this with your surgeon. A steroid injection or scar revision may be indicated.
