I'm scheduled for surgery in 2 months. I'm currently a 34B, 5'9", 130 lbs. Slender, tall frame. My surgeon has suggested 275 cc after I asked for a natural look. I have very mild sagging but opted out of adding a lift and am hoping the implants help a little with that. I'm becoming nervous 275 will end up larger than I anticipate but am also afraid that a 240 cc will be too small or not help with a "natural lift". I would love a second opinion. I'm at odds
Answer: Size There is a small difference between 270 and 240 cc's. with your height, I would select the larger implants. Both should place you in a full C in most bras. Ask to try both sizes on so you can see.

Answer: Implant size At size 34, each 100 cc of implant corresponds to 1 cup size change. 240 cc vs 275 cc will make a difference of about 1/3 of a cup, which will be imperceivable. Unfortunately, you have not provided photographs. However, you may benefit from a lift. More information needed to give a more accurate answer. Best Wishes, Gary Horndeski, M.D.

Answer: Don't let surgeons guess First, no one in the world knows what you mean by natural. It's a subjective descriptor that could mean very different things to different people. Second, patient dissatisfaction is almost NEVER due to breast shape, but instead breast proportion relative to the body. Finally, determination of body proportion is best determined by the person getting the surgery. The technique used to determine the appropriate implant size is called sizing, and it utilizes purpose built sizers of known volume to be placed into a tight, unpadded bra with a blouse or T-shirt worn. It takes all the guesswork out and gives the patient peace of mind so that they will wake up with the right implant size. If your surgeon didn't offer this to you, then you should find another surgeon.

Answer: Choosing implant size If the surgeon is having you make the decision, and you’re the one who has to make that decision. Without even including pictures, we can’t even begin to make an assessment regarding what may be appropriate. Each plastic surgeon has their own way of helping patients determine implant, shape, and size. In my practice, we work with lots of before, and after pictures and try to find a large selection of before pictures of patients who have very similar characteristics to the patient getting the procedure. Once a desired look has been established, we use sizers during surgery to determine what implant will create the image. The patient has described as being ideal. We always have a large selection of implants available during surgery to choose from, and the final decision is made during surgery using temporary sizers. This has worked well for us getting consistent results that are very predictable and match which patients have described as their desired outcome. He’s a plastic surgeon will have their own way of helping patients choose implant, size and shape. If your surgeon left a decision up to you and you’re the one who has to make the decision. There’s no possible way we can guide you with this decision based on the information provided. My best advice for anyone considering cosmetic surgery is that multiple in person consultations with local plastic surgeons, Welland advance of scheduling surgery. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patient who had similar body characteristics to your own. And experienced provider should have no difficulty showing you the before and after pictures of at least 50 previous patience. For common operations like breast augmentations highly experienced surgeons should have hundreds or preferably thousands of before, and after pictures to choose from. Being shown a handful of pre-selected images, representing the best results of a providers career is insufficient to get a clear understanding of what average results look like in the hands of each provider. You should also have a clear understanding of the most common reasons for surgeons, doing the revisions, what the revision rates are, and what their revision policy is regarding cost The biggest mistake patients make scheduling a single consultation, followed by scheduling surgery without taking the opportunity to vet multiple providers to help Find the best provider. The second biggest mistake patients make is doing virtual consultations. Best, Mats Hagstrom, MD
