This is a suboptimal facelift scar result/outcome in my professional opinion. There is some lateral sweep deformity, tragal distortion, poor incision planning, and hypopigmented (lacking color) widened scars. I would recommend considering revising this if it bothers you. I would ensure to find a reputable American Board of Plastic Surgery certified surgeon. Ideally, this surgeon has also completed an Aesthetic Society accredited aesthetic surgery fellowship. This will ensure your surgeon has the most amount of legitimate plastic surgery training and certifications.
The photo shows a certain amount of sun damage that pre-existed your facelift combined with hypertrophic scarring. These days, I generally combine fractionated CO2 laser resurfacing with a deep plane facelift. Currently, fractionated CO2 laser of the face and neck would help blend the scars and improve the sun damage.
does it NEED to be revised? technically... no. however, i'm sure you don't love how it looks. unfortunately this is the result of a lift that relied too heavy on pulling and tension and that's why you literally experience pulling and then the tension has dragged the incisions wide. it would not be very easy/feasible to fix this right now bc you probably had too much skin removed, and the only way to address this would be to somehow recruit more skin (that you don't have) to then cut out the old scars. best would be to see someone in person so they can feel your skin laxity and see if that's possible but I doubt it looking at the pics.
Ideally you would like to remove those white areas. That would depend on how much looseness there is at this point. Probably can get some worthwhile improvement
You lost some pigment due to interruption of blood flow. Hopefully, you do not use any tobacco products. I would suggest no further surgery now, but micro-needling for the scars and cosmetic, not artistic, tattooing for the areas where piment is lost. The pulling will slowly improve over time as your tissue loosens and stretches.
You have hypopigmented areas due to loss of the pigment in your skin from stretching the skin too tight. You're going to need an in-person examination to see if you have enough skin to see if it's worthwhile revising these incisions.
At 23, no facelift should have been done on someone with such tight skin. However, the pixie ear can be released and reset and this can be done under local. If it has been tried 4 times, you now have a lot of scar tissue in that area. although you do not want to hear this, it would be best to do...
I would suggest micro-needling for the scars and micro-needling plus radiofrequency to help the jaw line.
I do not recommend attempting a deep plane lift without general anesthesia because you will not have good pain control with just local anesthesia. I am not sure you will find a surgeon to do a deep plane with without a general anesthetic. I have done SMAS lifts with local and IV sedation and...
