I'm 43 / 5'8 / 136lbs. My surgery was 6/23 w/ Sientra: Silicone Mod+ 385/355 Smooth/Round. I shared my goal pics w/ my Dr, asked for smaller implants and said rippling was my biggest concern. He insisted on over the muscle. I truly dislike my result, they are too big w/ constant inner cleavage rippling. I want to avoid a lift if I can. What is a possible solution for my situation? And what type of implants might be better suited for my initial goal? To clarify my surgery was 6/18/23 and I'm currently considering going down 100cc I'd like to go down more but I've been told that is the most I can downsize without having to get a lift.
Answer: Ripples with 'Overs' Of course you have problems with implants placed over the muscle. The simplest solution would be to change your current underfilled Sientra implants with either Natrelle Inspira with their cohesive gel or, my preference, with Motive Round implants (the higher cohesive of the two gels they offer). This is not a sure thing, but ripples should be improved. Alternatively, get a more invasive revision surgery to place implants under the muscle, where I think they belong, even in young, athletic people.

Answer: Implants I think you will be happier going down about 100 cc's. Using gel implants gives less rippling. I prefer behind the muscle for better upper breast fullness, more support to the implants and less rippling. Being in front of the muscle , you now have more sag and might need a lift.

Answer: Removal of implants, circumareola lift with smaller implants At this time, I recommend removing the implants and doing a circumareola lift to move the nipple-areola complexes more medially. You will also need plication of the capsules inferiorly and laterally to place the implants higher and more medial. I also recommend the retro-pectoral position. A new pocket can be more stable then trying to plicate the old pocket. I recommend smaller silicone implants for less rippling than saline implants. Best Wishes, Gary Horndeski, M.D.

Answer: Breast implants too big & rippling, what is possible for my situation? I agree, your implants are too large I would recommend changing them to smaller implants and fat grating the skin to prevent rippling
