Aged 54. Skin around mouth and chin area poor. Have tried Obagi skin products/tretinoin for 3 months which reduced redness/skin tone, but failed to improve mouth and chin texture. I've had 3 light skin peels, fillers and botox in the chin crease but this did not help. What is the best treatment to drastically improve mouth and chin area? I can't afford to spend more money on treatments that have little/no effect. Profhilo has been suggested, but am reluctant to try if results are limited.

In the majority of people your age, the face loses volume which causes poor skin tone. Having fat transfer to the face will usually restore facial volume and improve texture of the skin. Answer: Treatment of Skin Textural Problems around the Mouth Without pictures or an exam, it is impossible to give specific advice. However, yours is not an uncommon complaint, and it gets more common the older you get. There are 2 main reasons for this. First the skin stops producing new collagen (the support layer main structure) and elastic tissue around 30 years of age. This causes the skin to thin and lose its elasticity that results in crinkles and lines as well as textural changes. Secondly, as the skin thins, the underlying muscles have more and more effect on the skin. Primary treatment of this is to thicken the skin and add elastic tissue. There are 2 methods to do the former, use of tretinoin or professional micro needling or fraxal laser. I prefer use of tretinoin, but 3 months is not adequate, as it takes several years to rebuild what you have lost over the last 20. The only product that replaces elastic tissue is Obagi's ELASTIderm twice a day. If you want to speed the process, then a full leveling peel to smooth the outside layer. "Light skin peels" and most other procedures that are presently offered are a waste of money. The nice thing about tretinoin and ELASTIderm is that you can treat the rest of the face, which will eventually develop problems like your chin if nothing is done.
