Most surgeons have a rule about wearing underwire bras. What it really comes down to is comfort and not applying too much pressure right on the incision when it is fresh. Generally I would have patients not wear an underwire bra for about 3 weeks. At that time I don't mind, particularly as I have my patients use silicone strips to minimize scarring. The combination of silicone and a bit of pressure works exceptionally well to help scars fade away.The other advantage to the underwire is that is shaped to contour around the implant. That can provide some support to keep the implant in the perfect location while the scar and capsule form around it. This is very surgeon-dependent. I like my patients to wait at least 3 months because that is the time period when the capsule is forming and I do not want wires to interfere with lower pole shape.
It can be tempting to want to rushright out and do new bra shopping after a breast augmentation, however I typically allowmy patients to resume wearing underwired bras at sixweeks because their scars are generally then reachtheir strongest point of resistance. At six weeks, I also allow mypatients to stop using their compressiongarments and resume normal exercise/physical activity.

Each physician has his own protocal which in many respects is determined by how the breasts look after surgery. In some patients there is no need to wait and in others it could be as long as three to six months. Ask your PS about his policy in your situation
For me it depends on two things. First, if I want to make sure that the crease level does not descend, I will advise an underwire earlier rather than later. Second is the scar location. If the scar is in the crease, an underwire can cause irritation/inflammation and so I advise waiting 4-6 weeks and then letting pain be the guide.
I agree that the implants are too far out on each side. To fix this you could have a capsulorraphy to essentially sew closed the outside portion of the pocket.
Yes, pain can be a symptom of the most severe encapsulation or capsular contracture. This can be fixed often with a capsulectomy and implant replacement.
I think that from what I see that you have a very good result. You start with some asymmetry that continues.
