What is happening? Incisions going nuts! Slough? Dehiscence? Please help! (Photo)

I am 3.5 weeks TT/Lipo/BR. All was well. My suprapubic area started opening a bit week 2, then belly button looked yellow. Both my drain sites are still open and have what looks like slough. My PS put me on abx preventative and AG dressing. No ointments. No debridement. Yesterday I woke up to my breast incision opened like an inch, with stitches coming out (can't reach them). He doesn't seemed concerned but I am freaking out! I am healthy, never smoked, no medical issues.
