I am so happy you showed your before pictures.  That is a tremendous amount of skin excess and after looking at all photos, I would have to say your result is very dramatic.  Could more skin have been removed?  Tough to make the call.  Maybe he or she was concerned about the blood supply to your skin with such a huge dissection.  Best to go back than do too much and have some really bad complications like gangrene of the skin.  I think your surgeon did a great job.With that amount of excess skin, you have to understand that this is not a 'one and done' effort.  Very common to require other procedures for further improvement.  Dr Spies

and when patients like you walk into my office, they are all told to expect revisions because its impossible to achieve a perfect outcome as your skin just keeps coming.  Often, a reverse tummy tuck is needed to smooth the upper abdomen.  Focus on your healing, know your surgeon did a good job, and know that revisions are needed and if your surgeon did not mention this, then that is all he is guilty of.  And it is unlikely the upper lateral roll will go away but that is what revisions are for.

Your result and contour look GREAT! It does not appear there would have been any additional benefit from going higher. Your other torso rolls will need to be addressed with different procedures - the FDL will not change those.
