Absorbable sutures can "spit" or become a source of post-operative irritation. Absorbable sutures usually break down without problem. Less often, the suture material can cause a local inflammatory reaction. Sometimes these clinically resemble an ingrown hair, pustule or pimple. Treatment for "spitting" sutures may involve lancing, oral antibiotics, or removal of remaining suture material with a fine forceps. I typically try to avoid using deep permanent suture material with most (if not all) septo-rhinoplasty procedures. Permanent sutures can become infected or extrude through the skin or mucosa. Absorbable sutures eventually breakdown, and spitting absorbable sutures usually resolve on their own. Still some can keep growing in size, with more inflammation and tissue distortion. Less commonly, they can lead to a more severe infection. It is important that anyone with a possible spitting suture, or suture abscess, see there ABPS board certified plastic surgeon. Early treatment can help prevent a more severe problem or infection.
