The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

I almost always place implants submuscular. It lowers the rate of capsular contracture significantly. In addition, it looks much more natural because the muscle provides covering over the implant so its not as round on the top. I've also noticed the implants drop less over time when they are protected under the muscle.

Because you need a lift and have no upper chest fullness, I would suggest placing the implants behind the muscle. With such a small implant, the lower profile should work better for you and not add much more volume once all the excess skin is removed. You could even consider just going with a lift and see if you like that size.

Your breasts have a narrow base that are mildly considered as tubular. So with the lifting operation you can get an augmentation with implant (the size may change according to your expectations). It is better to have the implants over the muscle for your condition to serve as a wider base for your breasts and help to treat tubular appearance.
